Clinical history
A 27-year-old woman presented to the outpatient department after watching several educational videos on breast reduction surgery, having already decided this was the procedure she wanted to pursue. She described longstanding macromastia – disproportionately large breasts relative to her frame, that had shaped her wardrobe choices for years. Fitted silhouettes such as spaghetti-strap dresses and halter-neck tops were uncomfortable or simply did not sit the way she wanted, and she reported ongoing self-consciousness around clothing that most women her age wore without a second thought.
During the consultation, she produced a bandeau top she had bought three years earlier and never worn, asking whether surgery could reduce her breast size enough to finally wear it. Her goals were discussed in detail alongside a clinical examination, and bilateral reduction mammoplasty was planned to address both the physical disproportion and the functional limitations it had placed on her lifestyle.

Markings were finalised for a reduction technique preserving nipple-areola sensation and blood supply, with the target volume set to allow comfortable fit in fitted and strapless clothing.
Excess glandular, fatty, and skin tissue was resected symmetrically under general anaesthesia, with nipple-areola repositioning and layered closure to support a natural contour.
